Assistant Trusted Assessor

Up to £28,054.79 per annum

NorseCare are recruiting for an Assistant Trusted Assessor to join the team at Head Office on a fixed term contract for one year.

As Assistant Trusted Assessor you will be supporting the team in ensuring the timely assessments of referrals take place on behalf of Registered Managers, supporting with the swift, safe discharges from hospital through working with social workers, the Adult Social Services Brokerage team, or from private individuals.

Main Responsibilities will include: 

  • Completing assessments of referred patients or members of the community for packages of care returning from hospital stays.
  • Alongside the Trusted Assessor, acting on behalf of the Registered Manager as a point of liaison for discharge teams, social workers, patients and their families.
  • Assisting the Trusted Assessor in reviewing the accuracy of assessment forms,
  • Supporting the Trusted Assessor in reviewing and managing referrals for Community Step Down flats and making recommendations accordingly.
  • Assisting with collating and providing accurate data regarding NorseCare’s occupancy levels to the Trusted Assessor and external stakeholders.
  • Reporting on issues raised by managers relating to quality of discharge and referrals to the Trusted Assessor.

Our Successful Candidate: 

  • Level 3 Health and Social Care qualification or equivalent working experience
  • Experience in assessing service users’ needs within health / social care setting
  • Previous experience of working with individuals with complex health / social care needs, long term conditions, complex comorbidities, or dementia
  • Demonstrable experience of working with older peoples’ services
  • Excellent communication and organisational skills
  • Full UK Driving Licence to meet the travel requirements of the role with the flexibility to travel as and when required
